Darrell Horn

During his work as a Patient Safety Officer and Critical Incident Investigator in Winnipeg, Darrell led or participated in more than 50 full-scale averse event reviews. He was certified as a Lead Critical Incident Investigator in 2009 and applied his expertise in a wide range of settings—pre-hospital care, acute care in-hospital and long-term care facilities. Darrell has also provided human factors assessments and undertaken critical incident investigations in rural and urban settings across Canada.

Along with conducting human factors programs for front line staff and education sessions for medical investigators, he has participated in various research projects and publications in the field of patient safety, collaborating with experts worldwide, including Dr. Sidney Dekker.

Darrell has an extensive professional background in commercial aviation, having worked as both an air traffic controller and commercial helicopter pilot and has been keenly interested in adapting the best practices of aviation safety into the field of healthcare. He is a World Health Organization Patient Safety Champion and was a founding director of Patients for Patient Safety Canada. He has published numerous editorial writings on patient safety, as well as offering guidance on radio and television and is currently working on his first book dedicated to the proposition of safer health care.
